Forgot your Carte Vitale during a doctor’s appointment? If your doctor gives you a paper feuille de soins, you can use it to claim reimbursement from French health insurance (Assurance Maladie). There are now two ways to submit your paper medical claim:

  1. Through the Ameli app: For eligible claims up to €100.
  2. By post to CPAM: For claims above €100 and other situations not eligible for online app submission.

Here is how both procedures work in detail.

If you have any questions, please feel free to use the comments section at the end of this article.

Submit your feuille de soins through the Ameli app

From Septermber 2026, A new service allows you to submit eligible paper medical forms directly through the official Compte Ameli mobile app, without posting the document to your local CPAM office

Who is eligible? Your claim must meet these conditions:

  • The consultation must be with a general practitioner (médecin généraliste) or specialist (médecin spécialiste).
  • The consultation must take place outside a healthcare facility, such as a hospital.
  • The amount on the paper form must not exceed €100.
  • You can submit a maximum of 5 reimbursement requests within 30 days, including claims for your dependants.

How to submit your claim?

  1. Log in: Open the Compte Ameli app on your iPhone or Android.
  2. Find the service: Go to Mes démarches → Feuille de soins – médecin.
  3. Upload the document: Take a photograph, select an existing image or upload a PDF of your paper feuille de soins.
  4. Identify the beneficiary: Indicate who received the treatment, yourself or a dependant.
  5. Identify the doctor: Enter the doctor’s 9-digit identification number printed on the form.
  6. Enter the amount: Provide the consultation fee, which must not exceed €100.
  7. Submit: Tick the certification boxes and validate your request.
  8. You will receive an acknowledgement in your Ameli account’s messaging section confirming successful submission.

Important:

  • Only eligible paper forms issued by doctors are accepted.
  • Blurry or poorly illuminated photographs may be rejected. You have a maximum of 4 upload attempts.
  • Once successfully submitted through the app, do not send the original by post. Keep it for 27 months.

Submit your feuille de soins by post

If your claim exceeds €100, or your consultation does not meet the Ameli app’s eligibility conditions, you must use the traditional procedure.

Step 1: Complete and sign the form. Check that the required information is filled in:

  • Your social security number (numéro de sécurité sociale).
  • Your name and address.
  • The patient’s details, if the treatment was for a dependant.
  • The required signature of the insured person.

If the patient is your child or another dependant, provide their details and the insured person’s information as requested on the form.

Step 2: Find your CPAM postal address. Send the document to the CPAM responsible for your health insurance. Find the correct address using the official Assurance Maladie postal address service. The postal address may differ from your local CPAM reception office.

Step 3: Send the document. Place the completed and signed original feuille de soins in an envelope and send it to your CPAM with prepaid postage. You can also deposit it at your local CPAM reception point.Keep a photograph or copy of the completed form for your records.

Step 4: Check your reimbursement. Processing times vary depending on multiple factors.

How long do you have to submit a paper feuille de soins?

Generally, you have 2 years to submit a paper medical form and request reimbursement from CPAM.

  • For treatment relating to an illness, the deadline expires at the end of the calendar quarter, two years after the quarter in which the treatment took place.
  • For example, for a consultation on 6 October 2026, the deadline is 30 December 2028.
  • Different rules apply to maternity-related reimbursements.
